This agreement helps ensure clear expectations, scheduling, and services tailored to your loved ones needs.
Service Agreement & Terms
1. Services Provided
Moore to Life Senior Concierge Services provides non-medical support including companionship, errands, organization, appointment support, and general assistance.
We do not provide medical care, medication management, hands-on personal care, or skilled nursing services.
2. Scheduling
Services are scheduled in advance based on client needs and Moore to Life availability.
Appointments are reserved upon receipt of payment.
3. Billing & Payment
Hourly Rate: $30/hour
1-hour minimum: applies per appointment.
Billing: begins upon arrival and continues for the agreed scheduled appointment time. Moore to Life will make every effort to complete services within the scheduled visit. Any additional time requested or needed beyond the scheduled appointment will be discussed and billed accordingly.
Additional time: If services exceed the scheduled/prepaid time, additional time will be billed in 15-minute increments.
Payment: Full payment is due at the time of booking to secure scheduled services.
Accepted payment methods: • Online: PayPal, Venmo, Apple Pay, or credit/debit card through the Payment page
• In person: Square (credit/debit) or cash
Refund Policy: All payments are non-refundable, subject to the rescheduling credit policy below.
4. Cancellations & Rescheduling
At least 24 hours’ notice is required to cancel or reschedule.
With at least 24 hours’ notice, payment will be applied as a service credit.
Service credits must be used within 15 days of the original cancellation date or will be forfeited.
Less than 24 hours’ notice, no-shows, or if the client is unavailable at the scheduled time will result in forfeiture of payment.
5. Overage Time
If a visit exceeds the scheduled/prepaid time, additional time will be billed accordingly and invoiced separately.
6. Transportation & Travel Fees
Local errands within a 5-mile radius are included in the hourly rate.
Appointments or trips exceeding 5 miles may incur a flat $15 travel fee per outing.
Service time includes all time spent assisting the client during outings, appointments, and transportation support.
7. Scheduling & Arrival Times
If the client is running late, please call or text (480) 749-0127 as soon as possible.
Service times may be shortened to respect other client appointments. Scheduled appointments are reserved in advance, and prepaid time is non-refundable except as outlined in the cancellation policy.
8. Liability
Moore to Life Senior Concierge Services is a non-medical service provider.
We are not responsible for medical care, health outcomes, or emergencies beyond acting in good faith to support client safety and notify emergency contacts or services as needed.
9. Emergency Protocol
In the event of an emergency, Moore to Life may contact emergency services (911) and notify the listed emergency contact.
10. Termination of Services
The client may terminate services with 7 days’ written notice.
Moore to Life may terminate services immediately, without notice, if circumstances arise that affect the safety, comfort, or ability to provide services appropriately.

By signing below, all parties acknowledge they have read, understand, and agree to the terms outlined above.
